Refunds:

-Full refund minus a $40 administration fee will be approved for any requests prior to Dec 1, 2009.

-Full refund minus a $40 administration fee for medical requests prior to Feb 1, 2010

-50% refund will be approved for non-medical requests from Dec 1, 2009 - Feb 1, 2010

-No refunds after Feb 1, 2010
